The one-page medicine list every appointment needs

Clinicians scan faster than they memorize. A clean list prevents “forgotten” over-the-counter drugs and allergy misses.

Large print (14–16 pt), two columns max, one page front only. Highlight anything new since the last visit with a yellow marker in the margin—your clinician’s eye will go there first.

Pharmacist tip

If you use more than one pharmacy, note which pharmacy fills which drug. That prevents duplicate therapy when a specialist adds something similar to what you already take.

Remember: do not stop or change prescription medicines without speaking to your prescribing clinician—some drugs need tapering.
